Human behaviour causes 99.5 per cent of accidents!

FACTS from accident investigation reveal that 99.5 per cent of accidents are caused by people’s behaviour, one US-based Facebook account video claims.

In fact, the latest Mengalum boat tragedy (one alleged it’s manslaughter, not even an accident) triggered another round of cynicism against the established order – what some called a ‘failed system’s’ generous tolerance for reckless cowboy-style behaviour.

Is Sabah ready to change where it really counts, the first cause – people’s behaviour, from top down, not just jetties, or boat quality?

Strike while the iron is hot, the saying goes.

So, for a change, we publish an instructive Facebook transcript for Sabahans to ponder while the tragedy of three dead and six still missing is still fresh on everybody heart.

Here it goes:

‘People cause accidents’.

‘To be more exact, people not paying attention or doing something that they shouldn’t be doing cause accidents’.

‘To put it another way, accidents result from varying degrees of carelessness, sloppiness, inattention negligence, neglect and ignorance.’

People create majority unsafe conditions!

‘Accidents also happen when two things are found – unsafe acts, unsafe conditions.’

‘Unsafe conditions occur when there is a failure to identify, control, remediate or train properly against hazards.’

‘Even though there may be multiple human causes, accident investigation will always identify the human failure to correct the hazards.’

‘Some unsafe conditions are easy to spot while finding others need a closer inspection.’

‘For example, oily or liquid on a walking surface is an unsafe condition but you won’t see it if you aren’t looking where you are stepping.’

‘People create the majority of unsafe conditions.’

Mother nature give warning signs…

‘Mother nature can create conditions but it gives us plenty of warning signs.’

‘If it is raining cats and dogs, wet, rivers swollen and flooding, we should be aware of the hazards.’’

‘No matter what nature is up to, we can be aware of it and can often guard against it.’

(One boat operator told Daily Express his company is famous for cruise cancellations, at his own cost.

How does he guard against nature’s fury? Employ one weather watchman on mainland beach and another on the island).

‘However, unsafe conditions created by people are often harder to find: Rampant speeding, flying motorcycles, poorly lit highways, absence of reflectors, overloaded circuits, (overloaded boats?) unguarded machineries, unchanged cylinders, altered safeties, blocked fire exits, blocked fire extinguishers, spills, poor lighting, unsanitary condition, damaged tools, inappropriately stored chemicals like gasoline, solvents and poor house keeping are all examples of unsafe condition, created by people.’
